I have only ever supplied my bank statement as my friends do not have accounts - this has never been a problem, the last application we made was in March 2011 so i would not assume that anything has changed. I have always showed 5k + in my account although from memory they look for $100 a day.
In regards to the form, perhaps complete the english version and email a copy of it through to your friend to copy.
