I asked the same question for my trip when i went over in March.
I got quite a few replies for contacts for drivers. We had 7 adults with luggage and we used Aussie Bemo who was excellent. It cost 50,000 total (not each) for all of us and he took us from airport to hotel in Tuban. Couldn't get better, he was fantastic and he confimed everything by email before i left.
If you do a forum search under my name Mandurahgal and look at my posts, think it was march this year and you will see a list of replies from others on the forum. I contacted a few of them and they were much more expensive. It was made who picked us up from Aussie Bemo, total cost was 50,000 and we gave him 50,000 tip
