supply a blanket but no linen for the cots. Yes everyone has to pay the visa on arrival. You will have a fantastic time in Bali, the Balinese love helping out with young children and babies. We have been a number of times now with our children and recently back (2 weeks ago) with our latest addition to our family our 4 month old daughter, we have always hired equipment from Bali Baby Hire so we didnt have to take too much, check out their website www.balibabyhire.com you wont regret it.
