VOA question and question re visas

During our upcoming trip to Bali in July my daughter and I are looking at doing a quick side trip (3 days) to Kuching (Malaysia) via Kuala Lumpur.

We are arriving in Bali on a Saturday and looking at leaving to go to KL on Monday morning and returning to Bali on Wednesday evening. On arrival in Bali we will obviously pay US$25 for our VOA but need to know if this will then also be valid for our return or will we need to purchase another VOA on Wednesday.

Can anyone also advise whether we will need to purchase a visa for KL and then also Kuching.

Any information you can assist with will be greatly appreciated.
