Phone's
Do they have to be unlocked? I have a couple of old phones locked to 3 here...I'm taking my iphone, but that is only going to be used for an absolute emergency!
Yes they do have to be unlocked to be able to place a Bali sim inside, there are websites that can tell you how to go about doing it for free, or take to Bali and get done, of course you can ask "3" to unlock for you but they will charge you. Check to make sure your i-phone is unlocked, if so all good.

Cakes
Breadtalk...are they able to specially make birthday cakes? Or do you just choose and then they ice names etc
They usually get you to choose the from the ones on show or from a book if at the end of a day and yes only add name. Check archives quite a few people have ordered from other places.

Airport
Is it really best to leave four hours before to depart? My youngest won't be happy, as it's her birthday that day!! We are flying out at 12.30pm.
You are quite close to the airport anyway, check the traffic when you are there, it will depend on when you are there.

And last but not least...we are staying in villas next to Best Western, and I understand that there is an internet cafe very near by. How safe would this be for my 15yo son to spend time there at night alone?

Only you can answer that one......I would have trusted my 15 yr old daughter but not my son hahaha, depends how street wise they are, how well lit the area is, will he be able to communicate with you whilst there if he gets into strife....
