My credit union charges no fees per withdrawal. The only thing that shows up on my account transaction lists are the withdrawals themselves.
The exchange rates I get are comparative to what I would get if I exchanged cash with a money changer - and no risk of being ripped off with wrong denomination notes or slight of hand etc.
I compare what I withdraw in Rp vs what shows on my statement in AUD$ and it's never been more than Rp200/$ different to the going rate at places such as PT Central and other 'non-commission' places.
Perhaps I don't get the full visa rate, but then I fail to see what that's got to do with anything when deciding if ATMs vs cash exchange is better value to me.
I do get charged a monthly account keeping fee, which is offset by a member rebate for money in savings accounts, loans etc - but I get these regardless because this is my every day card/account, so don't see why I'd take it into account with regard to use in Bali for one month a year. I'd be using it at home if I wasn't in Bali and would probably be getting slugged $2 ATM charges if I used other bank ATMs - which do show up separately on my statement.
