Wouldn't fly without them. I use them on the ascent and descent, they regulate cabin air pressure, and really do work.
Pinch your nostrils together while blowing down your nose gently and at the same time insert the ear planes in your ears. They are not uncomfortable to use.
As an additional precaution against ear pain i often take a couple of panadol 1hr before flying and ask cabin crew to advise me one hour before descent so i can take another couple panadol.
