A friend of mine who has been a lifelong smoker gave up about a year ago. After a while when going into a friend's place where people smoked he had one of those light-bulb moments and realized how terrible it smelt in there. Then he noticed how his clothes and his hair smelt when he left and how the smell lingers in his car even when people butt out before they get it. Then it hit him that this must have been the way he smelt and that other people hated the second hand smoke which he had never smelt while he was smoking.
Not a smoker myself and don't really like sharing other peoples' but in defence of smokers this incident with my friend was a bit of a light-bulb moment for me too. He just had no idea because he being a smoker he did not smell a thing! He thought it was a big fuss over nothing. Having said that he obviously knew the health implications because he stopped.
