Clothing sizing tends to run S, M, L etc in Bali, rather than in numbers. Unfortunately, there's no uniformity between sizes in different shops, markets etc. What fits in a size S from one place, may be too small at the next. I've always found that it pays to try things on, because even different items in the same shop or market stall can be sized differently.
I realise this doesn't help to you to shop for your wife, but short of taking her measurements and carrying a tape measure with you to check each item of clothing, there's not a lot more you can do since she won't be with you to try items on.
Shoes usually run by european sizing and again can be a bit hit and miss. I'm a 7 in Australia and in Bali find that anything from a 39 to 41 is what I need. I think they can be incorrectly labeled at times. Best bet might be to trace her feet onto a sheet of paper and carry that with you. You can then use that to see what size she'd need.
For yourself, you can easily just try them on to see what size you need.
