I don't think that was what she was talking about. Public loos in Bali are ruddy awful with no soap or water, the spray thingy for your nether regions is often covered in sh one T or pee or both.If adequate hygiene facilities are not provided then no one can use them.
The squat is not any more or less hygienic than a western loo, just different when used with the normal handwashing and regular cleaning of the facility.
