Our party of 4 really enjoyed our meal when we were there in June and my saltimbocca alla romana was as tasty as I've ever had...even better than the one I had at Ultimo.
If I was given stale bread I'd bring it to their attention.
If I didn't like what a restaurant served up to me I would complain and ask for something different.
I would have ordered some vegetables
and not everyone likes their pasta 'al dente'...I think it's nice to be given the option.
All of your grievances could have possibly been averted with a bit more communication with the wait staff and/or kitchen.
The fact that they didn't charge you for your meal was nice I agree and suggests that they're trying to do the right thing and perhaps not so deserving of such a scathing review.
