The fee depends on your bank and is not always $2. My parents were charged $4 on their NAB account no matter which ATM was used, Commonwealth included. My credit union charges me no fees at all from any Bali ATM.
The only ATM that gave me a balance in AUD$ was the Commonwealth one, but my parents didn't get the balance on their receipt on the same exact ATM when using it before or after me, so it must depend on your home bank for that as well. Other ATMs gave me either balance in rupiah or no balance at all, depending which ATM it was.
