I can recommend Melasti spa in Legian, or either Zen or Verona spas in Ubud. But there are literally loads of other fantastic places.
In terms of what you where, lots of places like the melasti get you to strip to your undies. If you dont have undies they give you disposables. If you are having some of the messier treatments in Ubud (scrubs etc) I have noticed that they are now giving disposables straight away. They used to get you to go buff and cover you with a towel, but I have only ever had that in Ubud and it wasnt sleezy at all just a way of not getting your undies not covered in whatever they are coating you in.
