A couple of years ago I had to take a young guy to BIMC after he badly dislocated his knee. He required surgery to put it back in place. His insurance papers where supplied to BIMC who sent through to the insurer straight away. Upon getting approval to proceed all bills went straight to the insurer and no money was paid upfront. I'm sure if you did not have access to the funds you could arrange the same if you had travel insurance.
