I have never had a problem with taking my panadeine forte (which contain codeine) into Bali either...mind you I have never declared it and I have never taken a doctors note. If you feel troubled, then ask your doctor for a note. I dont think they are going to bail you up as smuggling codeine in for a small packet of mersyndol.
