I've been using my Visa debit card from my local credit union in Bali ATM's since 2003 and never paid any fees on withdrawals - and always gotten the street rate, give or take Rp100 or so per dollar. Being a visa debit card it's always in positive balance so it's my own money I'm withdrawing, not a cash advance. It's just a regular ATM withdrawal from savings account.
I don't know if my CU is the last one standing but maybe you should shop around, there may be a local credit union or building society near you that doesn't charge fees. My parents use a credit union that is different to mine but under the same umbrella group and they do get charged per withdrawal.
