most of the local shops (warungs) as well as specialist stores sell them, you generally will see a board outside of the shop with the name of the carriers whose cards they sell ie XL, Telkomsel, Simpati etc etc,, they range in price from 35k to 50 k, this gets you the SIM with a local number and 5k credit, you should buy a further 30k credit minimum, the seller will charge 2 to 3 k commission for this.
If you get a simpati card you can call overseas very cheaply. you dial 01017 then country code (drop the plus sign and the 0) and then the number you want. Other carriers do similar with a different access no. Please note that most vendors are unaware of the access number required and that the documentation that comes with the sim card generally does not have this info either ( the card is generally in Bahasa)
