Commonwealth atms and the atms at the Carrefour Shop. Fees will depend on your bank at home. Money comes out in Rupiahs, card comes out last, don't forget it as it can be used straight away again from someone watching that you have left it.
Some atm's have limits on what you can withdraw, your bank will also have a limit on waht you can draw. We always managed to get one that withdraw 3 million rupiah at a time, to save on fees. The atm will ask you what language you want the transaction in before you start.
