We paid up front the hospital bill, but after calling International SOS I found out that if I hadn't been able to afford to do that they would have helped. They actually insisted on the upgrade, and as Qantas couldn't guarantee the bigger seat we had to fly Garuda. SOS made all the arrangements and paid, we just turned up at the airport. Our insurance was the free type you get when you pay on your credit card and it's underwritten by Zurich. SOS was really fantastic, but you do have to pay an excess of $200
