Best speak with your rheumatologist in Oz for opinion and they should know a regional colleague...if not I would be considering either a physician in Darwin or Singapore.
There is the International Hospital in Surabaya which would be comparable to Jakarta. Singapore would be superior to Indonesia and best to fly there before Jakarta.
The other people to ask is BIMC in Bali.
You may find that specialist treatment in Indonesia is expensive compared to a cheap airfare to Darwin and medicare entitlement.
