i was med evac'd out of bali a few years ago. flew to perth with a nurse. cost 15k AUD. luckily it was covered by insurance.
med evac is usually done on commercial aircraft and not a small charter. i think some ppl get confused by this. usually done by small charter in extreme emergencies. but this is not so common.
