Leigh gives good advices, I would also suggest the Amed area, Tulamben (where you can dive the famous USAT Liberty wreck) and Padangbai. Tulamben and Amed are close to each other and easy to combine.
Visa: There are three (afaik) different versions depending on which country you are from: no visa needed, just a valid passport; visa on arrival which you get at the airport and visa in advance which you will have to get before you enter Indonesia. If you are Austalian, you can get a 7- or 30-day visa on arrival.
