Usually a 3 part application process (unless already legally married before entering Australia - then it's 2)of forms etc which they can knock back at any stage.
Fiance Visa
Spouse Visa
PR
Final process of putting in application for Perm Res is 2 years after Spouse/Defacto visa is applied for/granted.
Medicare won't issue a temporary card until you are granted Spouse Visa (that is not on a Fiance Visa)
Once PR is granted then yes both names are put on a "proper" card.
