7,000 rp. When you buy a SIM card you can get it with any amount of credit that you want. This ranges from 5,000rp up to 100,000rp. When you use up this credit you simply wander into one of the numerous shops and purchase some extra credits.
If you phone happens to be locked and you are having hassles getting it unlocked go to the Bali Senia Hotel on Jalan Danau Poso(Sanur). Have a yarn with the Aussie owner Pete he will help you out. While there have a beer, they are always cold.
Pete is a wealth of information and can help you out with almost anything. His hotel is also one of the very few places around where the cricket will be telecast live.(the last bit may not be of interest to you however I threw it in for free.)
