I haven't been to Madura but often to Surabaya which is the place where you get ferry to Madura. From Denpasar to Surabaya is about a 30 minute flight with several every day. If by land then get overnight bus which goes on the Bali-Java ferry and then on to Surabaya. This about a 12 hour trip and not for the faint-hearted as the bus drivers are a wild lot. There are day buses too but slower and much less comfortable. From Surabaya it is a very short ferry trip to Madura.
