i have used er tours about four times over the years to do the northern highlights tour. each time we go to bali with diffrent people we take them on the tour. some one mentioned previously they are restrictive, we have not found this to be the case, in most instances if we have 4-6 of us we are the only people as they use 8 seater vans. i used them last october and they was only the two of us on the tour. the cost was about $28 aus each, the tour went for about 9-10 hrs and covered all the places you mentioned. just goggle er tours and you will find them.
