I recently checked with my travel agent about hiring scooter and my travel insurance while in Bali. The advice I was given was as long as I held an license from my home country and hired something under 200cc I would be covered.
The last 3 time in Bali I hire a bike to travel around and found it easier tan hiring a driver or taxi. The last time the hotel I stayed at got a bike delivered and did all the paper work.
The worst part about riding in Bali is I keep getting lost. The best bit is I have found some very interesting place when I have been lost which adds to the adventure
