I reckon designated means that instead of those people wanting to go nude on Kuta beach for example, they would go to the "accepted" area. I don't know the rules. I don't think that the locals that work on the beach are bothered by women that go topless on the beach as I have never seen any bad reactions to it. I have never seen anyone topless in a restaurant, and I think that is going a bit far. But if everyone is comfortable with it then OK.
From what I've heard from people that have gone to nude beaches, it is apparently quite "liberating".
