You will need
Kartu Keluarga (family card)
KTP (Indonesian ID CARD)
usually you have to apply at the local immigration office nearest to where you KTP address is stated.
For example my fiancee's KTP was registered at an address in Jogjakarta, he would have had to apply for his passport there, but then he re-registered and got a Jakarta address KTP and he was able to apply from the Jakarta office.
The fee for the actual passport itself only costs about 300 thousand rupiah, but with photos and 'agents fee" (if you use an agent) it usually costs about 500 thousand rupiah all up. If someone quotes you 1 million or more...find someone else!!
You fill out all the forms and leave it with the immigration office, then about 3 or 4 days later you return to get your photo taken, (get there early...its very busy and can be a very long wait) we waited 4 hours!!
Then you have a short interview on why you want a passport and what is your reason for travelling... after this you usually have to wait another hour or so...and finally the passport should be ready in your hands :)
