It does exist as some people in my hotel went there in May, 2007.
I was told it's at the back of the Kuta Galeria near Ace Hardware. They said it was good but extremely cold and were worried how your body would adjust if you stayed longer than 5 minutes.
Lots of locals visit and admission wasn't too expensive.
I hope my memory serves me right with the proximinty I've given you, no doubt someone will say so if not.
