4XL in pants is easily a size 26. I usually wear around a 22 and my 4XLs are extremely baggy on me. They fit in the waist as they have elastic, but the rest makes them look like clown pants on me. They're a lightweight cotton style, and they make them in shorts, 3/4 and full length. I ordered colours I wanted and they took around 2 or 3 days to make. I wish I'd tried them on first instead of just holding a pair up to myself though, I'd have got a smaller size.
In women's tops the sizing can vary with style. I bought two sleeveless non-stretchy longer length swing style tops. Very comfy. One is a 3XL and the other a 4XL. They fit like a 24-ish with the 4XL being slightly larger but not much in it.
5XL in a button up blouse I purchased would have been about a 28-30 and just looked silly on me. I bought it because it was really nice design and comfy fabric but I was way too hot to try anything on. Took a chance and lost out.
I'd say if you're looking for an 18-20, then head to the 2XLs but try things on where you can.
