Applying for a 60 day visa

I will need to apply for a 60 day visa for our visit to Bali on 28th August. I am from Perth and would like to know the procedure at the consulate, the fee, what I need to take with me, does my wife need to attend and any other relevant information.

My ticket shows us returning to Perth in 29 days but we intend to extend, will that be a problem?

I rang the consulate the other day and the phone just rang out, a touch of Bali in Perth I thought.
