I dont' recall seeing much in the way of fancy dress. Maybe the odd spiderman or superman costume (pyjamas?) for boys in the markets, but fairy dresses are everywhere. You can buy them at many market stalls for around Rp15,000 each. The quality is good. Or you can go to one of the many fairy shops around the place and pay a whole lot more for fairy dresses that are better in quality and usually much more nicely decorated. The market ones are lovely, don't get me wrong, they're just a little more basic with the beading/sequins. The fairy shops usually have other accessories too like wands and headbands etc.
There's a fairy shop on Jl Padma Utara, as saronggirl mentioned. It is, however, extremely overpriced for what they have. I bought some fairy dresses from a small shop on Jl Double Six in 2003, so don't know if it's still there. It was on the left hand side as you walk from Jl Legian toward the beach. She didn't have masses of stock, but all very well made, and from memory I paid around Rp40-50,000 each. The lady who ran the store handmade it all there.
I also recall seeing a fairy shop on Jl Legian, possibly on the stretch between Jl Melasti and Jl Padma. They had the sequined slippers, wands etc as well, and also made fairy dresses in various doll sizes, like barbie and baby born. I can't remember the prices as I didn't buy anything there.
