I might be wrong, but I'm pretty sure that if not all, most restaurants have a driver/guide room with free everything. Our driver explained this was the norm and that he prefers to have a break with his mates, traditional meal, TV to watch, and drivers / guides also get a commission from the restaurant for your meals & drinks.
He did add with a smile, that no driver would take guests to a restaurant that does not provide facilities or pay commissions.
Should add that I'm not talking about Kuta restaurants, I do mean places on the tourist routes.
I think to that most tourists would pay the day rate and add a tip if they are happy with the drivers service.
So no, it's not neccessary to pay for lunch.
But it's worth asking the question to make sure he's catered for.
