From the Garuda Office (at Kuta Paradiso) it was $450 AUD so we got it from the airport at Denpasar but you got to the Garuda info booth not the checkin guys. Couldn't do it from melbourne Airport. They do add the 1.5% extra for credit card. Not too bad - extra comfort etc - well worth it.
