I'm not sure who 'mdc74' is but I would have all the details of your child waiting to be sent out with your Receipt. The treasurer tends to do all of his paperwork at the end of each month (typical accountant), so it'll be out next week.
However, if you email me, I can tell you today. Just click on the my name above & remove the 'No Spam' part. However, the questionnaire & photo will still need to go out snail-mail.
