It does depend on the type of ticket and the flexibility of that particular class of ticket. We took a cheapy special last year and had to extend our stay due to illness. Our tickets virtually became useless if we did not catch the flight as booked and, as a result, we had to purchase 4 one way tickets home.
In our case our insurance covered the costs as the extension was due to illness for which we had a medical certificate.
