when we weren't happy with the accommodation and hotel, we contacted our travel agent who arranged for different accommodation options from the wholesaler the package was bought through(i.e. Qantas holidays, Bali on a Budget etc. If there was a difference in price (for us we decided to stay at a dearer hotel) we paid the difference in cash to the new hotel. The wholesaler/travel agent faxed or emailed the "voucher" (those confirmation vouchers that we all get in our travel documents) to the new hotel. Im not sure if I explained myself clearly, but I hope you get the drift :-)
