You'll need proper walking boots - they won't let you climb in sandals. Even climbing Batur was a difficult climb because of all the loose rubble and rocks. I took ordinary trainers and was slipping all over the place (fell over a few times and cut my arm open). Make sure you get boots with really good grip. You can always just tie them on the outside of your backpack. Get them in the UK cos you'll spend ages trying to find half decent ones in Bali.
All I took with me was a sleeping bag liner yeah - but even that was too hot most of them time. When you are under the mosquito net, you dont really need anything. Alot of the time I just used a sarong to cover myself up at night - you can get one on a market stall for about a quid. A liner could be handy if you are camping half way up Rinjani though.
