We can certainly recommend a great driver who works in CD.
Ketut is very safe driver ( a lot better than many of the taxi drivers), has a good command of English, and has a very good sense of humour which kept us thoroughly entertained during the trip. We have used him many times, and would highly recommend KETUT SUDIARTA as a driver. His rate is very reasonable at 400,000 rp for all day (includes petrol)0bviously much less if it's 1/2 day or less. You could email him and ask how much to be picked up from the airport and taken to CD. It may be may cheaper than taking a taxi from the airport (the other option).
CD is quite quiet, but has some wonderful places to stay that are very reasonable in price - you will have no problem finding a place. Personally I wouldn't book anything before hand, but rather have your driver stop at 3 or 4 places until you find the one that is perfect for you.
CD is also a good convenient base for visiting other places of interest in the area.

Not sure about the Spa - but I think most of the hotels have it available.
