The price for a guide was set at 300,000 per person when my friend and I did the climb last May. This includes a breakfast of boiled eggs/steamed bananas at the top of the volcano. When we did the climb, we arranged it with a driver, who sorted out the guide and a nice place to stay the night before. The hotel was in an area called Kedisan, very near the bottom of Mount Batur overlooking the lake, and was called the Hotel Surya. It was cheap (I can't remember exactly how much - possibly around 75,000 rupiah for a room) and included a very nice breakfast after the climb (the second breakfast of the day, but you need it!). There are no fans or air conditioning in the rooms, but it is so cool up there you don't need it. The staff were very friendly and were happy to cook up a good nasi goreng for us to eat when we were hungry in the evening.

I would definitely recommend doing the climb - it is absolutely fantastic being at the top of the volcano for sunrise.
