I had one done this last trip.... and have had so many complements on it, mine was done by Wayan at Mr. Dolphin in Kuta, opposite stadium cafe just up from Kuta Square. I can highly recomend him, he gives you the needle to open so you can see its new and sterile. I had mine done in the afternoon of the last day because i was told you can't swim in the pool after you have it done, although Wayan said it was fine too, just not to swim in the sea. Bit uncomfortable flying home though, as it's on the lower back.
