I have stayed in a couple of places with open air bathrooms. It is lovely to shower in the sunshine, but not so great when you are trying to do your make up and it is pouring with rain! Usually they cover the little bit where you sit on the toilet or stand under the sink so that you don't get wet. I remember a one incident though where I went into the bathroom to find my friend (a little tipsy!) asleep on the floor - in the pouring rain!
The other thing is the mosquitos - not just in the bathroom, but if you don't the inbetween door closed all the time, in your room as well. Expect geckos too - but then, we had them in all of our rooms, as wel were backpacking. They are harmless. We started to name them!
We did have one bathroom in lombok where I could see a second floor open window when I was in the shower - slightly concerning, but never saw anyone peeping out of it!
Overall, they are nice if the weather is nice, but perhaps just a bit of a novelty in the long term as some of the practicalities (epecially the rain and mosquitos) are a little annoying.
