I have visited several orphanages, christian and hindu, one in particular in outer Denpasar had wall to ceiling donated clothing and school books - it was crazy, too much stuff yet not enough money for school fees. Another had excess clothing and toyes but they did donate to their local villages.
Unless you are going to an orphanage a long way away, who dont appear to be as well visited by tourists, perhaps you may like to give the pre-loved clothes to your hotel staff or beach ladies for their children or for them to distribute around family and friends. Nothing is ever wasted it will get to someone who needs it. With the lack of tourists they must be finding it very tough too.
By all means, visit an orphanage if you like, however from my experience cash is probably a better gift to give. Most keep a book that you can record what you donate if you are into that.
