We have used this system for a couple of years now and found it to be very good. ATMs with a Cirrus or Maestro symbol can be accessed in Bali for a relatively small fee. We used Westpac until recently when they started charging a $5 monthly fee just to keep the account! We have just changed to an ANZ Access Select account - no monthly fee, $5 overseas ATM withdrawal. It is a joint account with 2 cards, one goes to Bali with a trusted person, the other is used to top up funds. You can SMS the PIN after the card has been delivered, your friend can SMS you when he has received the card. They can't take out more than you put in and they aren't going to spoil a good thing. In fairness, don't have more than a few dollars more than the agreed monthly withdrawal. Exchange rates are similar to Kodak street rates.
Hope this helps, just remember to watch the card expiry date.
