I've been called a know all before now but I have a theory on this (like everything else) Many people who work in cafes and even hotels do not and never have had refrigeration at home, and thus over estimate keeping times in a fridge.
The warungs and carts buy and cook every day fresh as is the traditional way in Indonesia. I think it's likely that tourists unused to spicy food get the runs from the chilli. The carts have their regular local cutomers and wouldn't keep them if they were handing out real full blown food poisoning which is what Bali Belly is.
However, if anyone feels at all unconfortable with the carts I would never suggest they try them. You need to feel relaxed to enjoy your holiday after all.
