I just received mine today for the LA Embassey. They are bad about answering the phone, ask for the Konsulur, instead of trying to go through to Immigration and visa info, that was the only way I could get through.
What you need to do is down load the forms off the site http://www.kjri-la-net/...fill them out, attach two passport photos, and write a letter on your own behalf, stating that you will be responsible for yourself. Enclose a copy of you ininerary/airline reservations, and mail a money order...took me almost a month to get it this time, which I felf was very long, but I got it....that's the main thing. It is considered a social/business visa...you do not need to have a sponsor, as the form says, just a letter from you....
The lenght of time they will issue the visa for is a max. of 60 days, however, if when in Bali you go to immigration, I believe they are at the airport, pay the fee, I believe it is about 350,000rp, go about 1 week before your visa ends, they will extend it for you....up to 6 months, total, but you must do the extenstion everymonth, and pay..
Be sure that you have a ticket for 60 days that you can also extend, because they look at that too...if the ticket is for longer than your visa it could create a problem...The San Francisco Embassy charged less for the same visa as did the LA Embassy..I's suppose to get a refund...yea right, checks in the mail...but who knows, I may get a surprise.
