When we were there in January I went to Denpasar and got a license there, paid 150 000 for a month.
You need to take passport and int. license with you. My int. license notes that it isn't for a motobike, it doesn't matter though you will still be issued with one.
BTW riding around the island (not just Kuta Legian and Tuban) is ALOT of fun and not really as hard as it perhaps might seem.
