In some of the places, the drivers and guides are not actually allowed into the restaurants, they get fed while they are there from another area or buy something from one of the street vendors.
As mentioned before some of the get commission, some of them don't. If you get a good driver and a guide, slip them some extra money. Remember that their average wage is about $60 australian a month!!! It won't hurt to part with some of your rupiah if you think they are fantastic.
