Both my kids got shocking ear infections and the hotel called a Dr, he was great, he flushed my son's ear out as it was full of wax, and came back the next day to do a follow up. He only charged me for my son, as I said to him, my daughter is feeling the same, he checked her and just said get her the same medication.
I always take over 2 pcks of Anitbiotics for such times, but they got that sick that I really needed a Dr's advice, and I was satisfied and our Travel insurance covered the 1 million bill
